Completing  New Patient Forms

Prior to your initial visit, we will need to gather important information from you to ensure we give you the best quality care. You will be sent a text message or an email with a link to complete the necessary forms. These forms are also available online click here!

What To Expect At Your Initial Visit

When it is time for your appointment, one of our friendly team members will escort you back to a treatment room. If you do not have a current set of x-rays, we will begin by taking a complete set of x-rays and also intraoral photographs of any issues or concerns in your mouth. Using all of this gathered information, one of our dentists will complete a comprehensive exam and determine the health of your teeth and gums.

A thorough oral cancer screening will be done as well by both the doctor and the hygienist. During your hygiene visit, the hygienist will use the VELscope as a courtesy to our patients. The VELscope is a light we shine into your mouth to enhance our vision to any abnormalities that may not be visible to the naked eye. This light is extremely safe and there is no radiation emitted.

After your comprehensive exam, Doctor will determine what type of hygiene appointment is best for your oral health and if any treatment is recommended. During your hygiene visit you will be taken care of by one of our highly skilled and gentle hygienists. During this time, she will be able to answer any questions you may have and help you to achieve your optimal oral health.

Reviewing Your Financial Options

At the end of your visit, if there is any treatment recommended one of our treatment coordinators will be happy to review the financial aspects and schedule any necessary appointments. Please visit our Financial Page (can we make that a hyperlink?) to learn about our different financing options.

We love welcoming new patients to our dental family and we can't wait to meet you!
